On the 1st of December, [redacted], there was an attempt to cash a check to buy $25.00 worth of Abt stock. My checking account was compromised, and I informed a representative not to proceed with the transaction. Despite this, the check was still cashed on the now-closed and compromised account, resulting in the sale of $25.00 worth of stock. I kindly request to have the stock amount reinstated into my account without any penalties, as the check was processed after the account was hacked and closed.

In June, following my husband's passing, I contacted your office regarding his ExxonMobil and AT&T shares. I completed the necessary forms to transfer the shares to my name. Despite receiving letters requesting a letter of appointment, the probate court only provided a bill designating me as the fiduciary. I was advised by a representative 12 days ago that an Affidavit of Loss was needed and would be sent promptly. Unfortunately, I have not received the form yet, leading to confusion. I am dissatisfied with the responses received so far and require assistance from a knowledgeable individual. The lack of progress has been distressing. I previously worked at Mulesoft in the UK before leaving and relocating to Germany. When Salesforce acquired Mulesoft in [redacted], I was unable to complete the "Letter of Transmittal" as the company did not have my updated address, despite having my email on file. Upon realizing I needed to engage with Computershare, I reached out via phone and email to request the letter be sent to my new address in Germany. Their response indicated that address changes could not be processed via email and directed me to contact an investor services representative or make the change online through the Investor Center website. When attempting to change my address online, I encountered an issue as a non-US resident because a "Holder Account Number" was required, which I do not possess. I am concerned about the inability to speak with a representative directly to verify my details. I urgently require assistance with this matter.

My brother set up an online account for our elderly mother, Virginia M., and accidentally locked us out of it. We have successfully transferred her bank accounts and Schwab account into her newly established trust. I want to help her access it to include her Computershare holdings in the trust and have dividend checks deposited into her Huntington Bank savings account. Due to safety reasons, I can't do this alone. I live far away, so online access would be ideal. However, being locked out is preventing me from assisting her. Thank you for your prompt response.

I recently received a letter from the CA State Controller regarding unclaimed property from INVENTRUST PROPERTIES CORP INVESTOR SERVICES COMPUTERSHARE under my name, Brian Barnett. The property is SC12 Underlying Shares/Outstanding Cer. with an Account Number of C[redacted], consisting of [redacted].00 Shares. Despite attempts to contact them, I have been unsuccessful. I called their customer service line but was unable to get more information as I kept getting disconnected. I live at [redacted] Red Sage, Irvine, CA. I am a co-beneficiary of a Revocable Trust, along with my deceased brother, managed by Computershare. Unfortunately, my brother has passed away, and I need assistance in understanding the process to access and distribute the shares from the accounts. The accounts are held at Computershare's offices in the Philippines, and I am finding it challenging to communicate clearly with their representatives due to their accents and scripted responses. The trust is designated as the Payable on Death (POD) beneficiary for the accounts, which contain shares in ServisFirst Bank and Proassurance. My late brother's name is David Alan Davis and I am Richard F. Davis. The accounts are under the name of "The David A Davis Revocable Trust" with the address [redacted] SW 14th Ave., Cape Coral, Florida [redacted]. Since September [redacted], I have been trying to resolve an issue with Computershare Louisville. Despite many phone calls and letters, including reaching out to the department head, all I received was another W-8Ben form with no explanation. I have already completed and returned this form. I am once again requesting a D.R.S. statement so my 70-year-old husband can finally cash in his shares. We never had bonds, and we are unable to proceed without the statement. I am now reaching out to CEO Stuart Irving as a last resort. This situation has caused financial distress for a pensioner who invested his savings for retirement. Computershare's lack of assistance is appalling. I hope no one else has to endure this nightmare.

Subject: Issue with Share Transfer I have made multiple attempts to contact your team regarding the mishandling of my shares in the ESOP plan I had with my previous employer, Advanced Drainage Systems, Inc. It seems there was an error in transferring $11,[redacted].51 to my current plan with Great West Trust Company, as only $8,[redacted].23 was received. The shares apparently got routed to Principal instead of Computer Shares as expected. I am requesting immediate assistance to resolve this issue and receive the missing amount. Good morning, I am Tom Guszkowski, an estate planning attorney with von Briesen & Roper, s.c. in Milwaukee, WI. I am assisting Anthony and Laurel Sikorski, an elderly married couple. They own 7,[redacted] shares of The Stanley Works and submitted a Transaction Request Form in March to deposit their shares into their reinvestment account. However, they have not received confirmation that the shares were deposited. Computershare Customer Service informed them that their forms were not received. I recently received a letter stating that my immediate action is needed per Georgia Unclaimed Property Laws. The letter mentioned a check for $39.36 from SunTrust Banks, Inc. It seems to be an old payment. The number provided, [redacted], was not working when I called. The account number mentioned is C[redacted]. The address is correct, and I just need assistance with re-issuing the payment if possible.

I've been locked out of my account after answering my security question incorrectly too many times. I am an individual investor and the question was, “What is my favorite animal”? I answered “Dog,” but now I think it may have been looking for a specific dog’s name instead. Your system doesn't allow me to correct this online. I am George W Cooke and my wife is Arline M Cooke (nee Ochs). We have encountered technical difficulties accessing our account in the past. Today, when I provided my birthplace (Eaton Rapids, Michigan) as requested, it was rejected. We hold shares in various companies such as JP Morgan Chase & Co., Carrier, Con Edison, Verizon, AST, Eebtec, and Otis. Our Verizon holdings are under C[redacted] and C[redacted]. The shares are under the name of Arline M Cooke and the George W Cooke family trust. I have attempted to access our accounts with Computershare using my ID and password with no success. Hello, I requested Fineco Bank to transfer some of my shares to you. They asked me to provide more information for the transfer request. The details needed are the departure account, destination account (Header, dossier, IBAN), any further useful data, and the securities to be transferred including ISIN, quantity, and name of the title. I seem to be missing the destination account details like Header, dossier, IBAN. Here is the additional information required: 1. Complete name of the Transfer Agent: Computershare Investor Services. 2. Tax ID Number (TIN). 3. Exact registration on the Computershare account, or if it's a new account, the registration as it appears on the brokerage account. 4. Exact number of whole shares to be transferred. 5. Full CUSIP number. Note: Computershare’s DTC Number is not needed. Matching registration, address, and SSN/EIN will go to an existing account, otherwise, a new account will be opened. From Italy.

I have been attempting for 9 months to sell [redacted] shares of stock through CompuServe. Despite having legal documentation proving I am the sole executor of my late father's estate, I have been informed that I must replace 2 stock certificates, transfer the [redacted] shares from my father's name to mine, and then they can be sold. I have made over 20 phone calls and submitted various documents multiple times, including fees to replace certificates and transfer shares, yet there has been no progress. Each time I call, I am unable to speak with the same person twice, there are no direct phone numbers or employee identification numbers, only first names are given. I have to repeatedly explain the situation from scratch on every call and receive conflicting information from each individual. Despite promises to review the case and return calls, no one has ever followed up. Today, I received two letters with conflicting instructions and one even requested additional fees. This issue is causing delays in closing my father's estate, and I am frustrated by the lack of progress. I simply want my father's [redacted] shares of stock sold so I can finalize the estate closure. I do not require physical stock certificates with my name on them. I find it perplexing that a company named COMPUTERshare operates in such a paper-intensive manner when they can easily confirm ownership electronically.

I, Richard P. Jessen, along with Junko I. Jessen, jointly own [redacted] shares of C.P. Pokphand Co. Ltd. under account number C[redacted]38 JNT. We are experiencing difficulty logging into the Investor Center despite having the correct login details. Although I followed the advice to delete cookies, I still cannot proceed after reaching the stock registration screen. This is puzzling as I registered my stock years ago and accessed the account two years back to update my address. The recurring prompt to register the stock, preventing me from accessing the Investor Center, leads me to seek assistance with logging in and ultimately selling the stock. Your guidance on why re-registration is required, navigating past the registration screen, obtaining active login support, and facilitating the stock sale process is greatly appreciated. I am involved in the InPlay/Prairie Storm transaction. On November 23, we completed stock transfer forms at the Computershare counter in Toronto. Computershare reassured us that new certificates would be issued and delivered to us by November 27. Despite numerous emails to Jacky Patterson and David Cavison requesting an update and tracking numbers for our certificates, we have not received any communication after Tuesday evening. Jacky mentioned contacting David for information. Could someone provide us with an update on the status of our shareholdings from the transaction?

I am having trouble accessing my account due to being locked out. Upon reaching the age of majority, I underwent a stock transfer process. Prior to the transfer, I believed I had to set up an account and did so with the username Klandwehr32, holding stocks in Verizon. When attempting to log in, I could only answer one security question instead of inputting my known password, leading to the account being locked. Unfortunately, I am unable to create a new account since it is connected to my social security number. I am uncertain whether I should delete or recover the current account, and if having this account is necessary for the stock transfer process or if creating a new one is an option. Any guidance on resolving this issue would be appreciated.

I have been experiencing ongoing issues with receiving an account access code through mail, with promised timeframes of a week and then 10 days passing without receiving it. This has been a frustration for about a year now, and after finally receiving a code months ago that was deemed incorrect, I am now locked out of my account. As a grieving widow, I find it incredibly distressing to deal with such prolonged issues. The customer service provided to address this matter is drastically lacking, failing to follow through on their commitments.
