Hello, **I have a Long Term Disability Claim with Cigna, Incident ******* and must have up to date examinations and records turned in by mid February. Physical, neurology and mental health.**I am asking for an extension until *st April. I pray for an extension, please read my request below. **My doctors are unable to evaluate me and*or complete tests and examinations until the end of March. I just had an EEG, but will not be able to see my Neurologist until mid March to discuss the results, and plan my treatment. I have other evaluations scheduled as well. **My doctors have been treating me, and can provide more in depth up to date information as to my condition, and is the preferred provider information for Cigna to base its decision(s) on. **My agreement with Cigna, plus Louisiana law guidelines clearly state that my own doctor's opinions carry the majority weight in disability decisions, and it would be arbitrary and capricious not to give my doctors and professionals the chance to give their very valuable input and knowledge into Cigna's decision, and me the right to have a reasonable amount of time to provide my records to Cigna. **I am making a respectful request first to my caseworker, Celena Lukasik, that I have until April *, **** to submit all required paperwork that Cigna has asked for. She has stated that she cannot make this decision and has to ask her supervisor, so I ask this of her supervisor, respectfully, as well. Since time is running out, I am urgently and respectfully asking Customer Service to please take this request and consider it, and give me until April *, **** to submit all my required materials. **concerning an extension as to my submission of disability paperwork submission. She has stated that she is not able to make that decision. Thus, I am turning to Customer Service to ask for consideration in this matter. *I am respectfully requesting that I be allowed to turn in my documents and test results on or before April *, ****.
