in **** i agreed to try aflac for a year and wrote a check* to my suprise years later i find without my consent or authorization aflac has been making direct withdrawals from my banking account for services that i did not agree to* i am told i need my policy number to stop this stealth of my funds-problem is i don't think i was given an insurance card back in **** (or did not keep it)- a policy number would be helpful please and i am attemtping to fax the signed and dated stop and refund request forms as directed by aflac-thanks for any help you can provide
